Abstract

Background SGLT2 inhibitors are a group of medications that are proven to reduce cardiovascular events and heart failure hospitalisation among patients with heart failure (HF) regardless of its ejection fraction (EF). Objective As SGLT2 inhibitors are being given Class I indication by major cardiology societies for the reduction of major cardiovascular events and heart failure hospitalisation, we conduct this study to characterise the rate of its prescription among patients admitted for HF. Methods This single centre, retrospective cross-sectional study was conducted in a tertiary university hospital among all patients admitted for heart failure between 1st January 2024 and 31st December 2024, regardless of the ejection fraction and diabetes status. We assessed the rate of SGLT2- inhibitor prescription among this cohort of patients. Results Of the 518 patients admitted during that interval, only 44% were prescribed SGLT2 inhibitors. Among those not prescribed, the main reasons were due to its contraindication from advanced kidney disease as well as financial constraints. Among those prescribed with SGLT2 inhibitors, majority (47.3%) were those with heart failure with reduced EF (HFrEF). Patients with HF with preserved EF (HFpEF) constituted 35.1% while patients with mildly reduced EF (HFmrEF) were 15.3%. There were a total of 124 re-admission within 90 days (23.9%) among this cohort, of which, 52.4% were those with HFrEF. Contrary to other studies conducted among patients with heart failure, we did not find a significant difference in re-admission rates among those prescribed with or without SGLT2 inhibitors (25.4% vs 22.8%, p value not significant). Conclusion The SGLT2 inhibitor prescription rate among patients with heart failure in our study was comparable with other contemporary studies. However, we did not find any significant difference in the rate of 90-day re-admission among patients with heart failure who were prescribed SGLT2 inhibitors compared to those without. Further study is needed to clarify this finding in our study population.
